A Clinical Study of Fibrin Sleeve Formation on Indwelling Subclavian Central Venous Catheters


Abstract
From April 1990 to December 1990 in 72 cases on indwelling subclavian catheters at the department of surgery, Hanil hospital, subclavian catheters injected with contrast material during removal were studied by cinefluoroscopy.
The cinefluoroscopic studies revealed that totally circumferential fibrin sleeves consistently and extensively formed on indwelling subclavian catheters.
Fibrin sleeves were detected by cinefluoroscopy in 70 cases(97%) of 72 cases.
In 70 cases over 6 days in the duration of the inserted catheters, fibrin sleeves were detected.
The rate of fibrin sleeve formation were proportional to the duration of the inserted catheters.
After the cinefluoroscopy, tip of the catheters, subcutaneous segment of the inserted cathters, blood in the catheters were cultured. And in 9 cases of 72 cases, the results of the cultures were positive(12%).
These 9 cases were the cases of the fibrin sleeve formation.
These fibrin sleeves showed possibility of the source of infection of the catheter sepses.
Therfore, we should concern warrantly about the management of the central venous catheters because the high rate of fibrin sleeve formation according to the duration of the inserted catheters and catheter associated sepsis.
